Pharmacy First – Resources
Our Pharmacy First Counter Toolkit has been developed to help contractors and pharmacy teams with the current Pharmacy First clinical pathways, following the October 2025 gateway updates.
The toolkit is designed to support the whole pharmacy team at the counter. It can help staff identify patients who may be suitable for Pharmacy First, ask the right initial questions, and make the handover into the Pharmacy First consultation smoother.
The toolkit covers all seven Pharmacy First clinical conditions. For each condition, there is a Gateway & Assessment Guide, a Counter Staff Guide and a Patient Completion Page. We have also included a briefing document and a Red Flag Quick Escalation Reference Guide to support safe and consistent use across the team.
These resources are local support tools only. Please always refer to the current NHS England Pharmacy First service specification, clinical pathways, Patient Group Directions and protocol as the formal reference documents for service delivery.
