Incident Reporting

Pharmacies must have a patient safety incident log for all incidents. Pharmacies should also maintain a record of errors identified as part of the checking process (near misses) for internal review and learning.

Patient safety incidents must be reported to the Learn from Patient Safety Events Service (LFPSE). Note: near misses do not need to be reported to LFPSE as a matter of course. The National Reporting and Learning System (NRLS) remains in place for patients and families to use to report incidents themselves.
