CD Reporting

Reporting of all incidents should be inputted onto the online monitoring tool www.cdreporting.co.uk.  To begin the process, all reporters of controlled drug concerns incidents will need to register on the website www.cdreporting.co.uk.

CD Destruction

Requests to arrange for an authorised witness for the destruction of expired Schedule 2 controlled drugs should be inputted onto the online monitoring tool www.cdreporting.co.uk.  To begin the process, all requestors of an authorised witness will need to register on the website www.cdreporting.co.uk.

CD Information

The CD Accountable Officer for NHS England (East of England NHS England and NHS Improvement)  is Jane Newman. Any issues relating to controlled drugs and medicines governance, pharmacy, and prescribing should be directed by email to england.ea-cdao@nhs.net.

Following the re-organisation of structures post the ‘Creating the new NHS England’ process, and the successful appointment of Bhavisha Pattani to the role of Director for Patient Safety & System Improvement in the Midlands, there have been changes to the ICBs covered by our CDAOs.
 
From 1 September 2024, the following arrangements will be in place:
 
East Midlands:
Samantha Travis will be the CDAO for East Midlands covering the following ICB areas:
 
Leicestershire, Lincolnshire, Northamptonshire, Nottinghamshire and Derbyshire.
